Effective Monday, January 9, 2023 we will be operating our Winter Crisis program.  The program ends March 31, 2023 or when funds run out.   This is a Crisis Program so you must be subject to disconnect or have a late notice.   You may receive help with your electric, natural gas or propane bill.   If you need assistance with your propane bill, you will need a letter from the propane vendor stating that your account is in good standing. 


 All of our Family Service Centers (Bullitt, Shelby and Spencer) are now providing in person services, however it is required that you make an appointment.   We do not want to overcrowd our lobbies.  At the date and time of your appointment, please call the office number that will be posted on the door and the appropriate staff person will let you enter and complete the application process.    Masking is at the discretion of the client.   

List of Docs to Provide

Here's the list of required documentation to either upload prior to your appointment or bring with you to your appointment.    We will need all required documents to complete your application.   If you do not have the proper documentation, you will be asked to reschedule your appointment.   If you miss your appointment time, you will be asked to reschedule your appointment.   

    Social Security card(s) or official documentation with social security numbers (or Permanence Residence/Green Card) for everyone living in the household.

    The copy of late or disconnect status for the utility bill that you want paid.  Or if you are prepaid plan, the statement for your pre-paid plan. 

    Proof of household income from the previous month including these examples:    Check stubs (MUST have all check stubs showing name of employeepay dates and gross income for the calendar month PRIOR to your appointment date)

  • OR, If you receive Social Security or Disability benefits, you MUST provide your current Social Security/SSI Award Letter.   

  • OR, Unemployment Summary of Benefits

  • If you had no income in the previous month, zero Income can be verified with our Zero Income Form which can been found at the following link:    https://www.mpcaa.org/uploads/1/2/9/8/129829946/no_income_general_3_county.pdf.    

The form from our website must be completed by someone that knows your situation, but cannot be anyone who lives in your household or is related to you.    

  • Examples of items which cannot be accepted are previous LIHEAP applications, bank statements, or copies of benefit checks, SNAP Benefit Letter, and 1099 tax forms. We cannot accept Medical Coverage Letters as proof of other income.

 The gross household monthly income for LIHEAP Subsidy must be at 130% of poverty. For each additional household member above 8, add $393.00 to the monthly income limit.
