All of our Family Service Centers (Bullitt, Shelby and Spencer) are providing services by phone or virtual appointment. We are not conducting in-person appointments at this time. You may schedule a phone appointment to receive services at these locations, and one of our staff members will give you a call at your scheduled appointment time.

List of Docs to Provide

Here's the list of required documentation to either upload prior to your appointment or to drop off at our location in the conveniently located drop boxes outside all of our buildings. If you are dropping off your documents in person, please wear a mask when you approach our door. No services will be provided if you are not wearing a mask. We will need all required documents to complete your application. Please make sure that you upload your documents or bring them by the offices prior to your appointment date and time.

  • Social Security card(s) or official documentation with social security numbers (or Permanence Residence/Green Card) for everyone living in the household.
  • Most current gas or electric bill. Bill must be at least one day late or subject to disconnect or you may also be on a payment plan. If pre-paid account, we would need letter showing that account has 10 days or less left for services. For propane/kerosene, household would need to be within 4 days of running out of fuel or your tank is registering at 20% or lower.
  • For clients who use propane/kerosene as your heating source we will need a letter from the vendor stating that you are a client in good standing and must be within 4 days or at 20% of running out of propane.
  • Proof of household income from the previous month including these examples:
    • Check stubs (MUST have all check stubs showing name of employee, pay dates and gross income for the calendar month prior to your appointment date)
    • OR, If you receive Social Security or Disability benefits, you MUST provide your current Social Security/SSI Award Letter
    • OR, Unemployment Summary of Benefits
    • If you had no income in the previous month, you will need to pick up a zero income form at one of our offices prior to your appointment. The form must be filled out by someone that knows your situation and cannot be related to you or live in your household. Or you may download a copy of the form at our website at this link Zero Income Form
  • Examples of items which cannot be accepted are previous LIHEAP applications, bank statements, SNAP Award letter, copies of benefit checks, and 1099 tax forms. We cannot accept Medical Coverage Letters as proof of other income

Multi-Purpose Community Action Agency (MPCAA) seeks to reduce poverty and help low-income citizens of the Bullitt, Shelby and Spencer Counties to meet critical needs and become self-sufficient.
